I understand that the tattooing process involves the use of needles and may cause pain, discomfort, and potential risks, including but not limited to infection, allergic reactions, scarring, and undesired results. I acknowledge that Mayhem Ink Tattoo Phuket, has explained the potential risks and complications associated with tattooing, including proper aftercare procedures, and I agree to follow their instructions diligently. I confirm that I have disclosed any medical conditions, allergies, or medications that may affect the tattooing process or my health, including but not limited to skin disorders, blood-borne diseases, or immune system disorders. I understand that it is my responsibility to provide accurate and honest information regarding my medical history, and I release the tattoo artist and the tattoo studio from any liability arising from any undisclosed medical conditions or inaccurate information provided by me. I acknowledge that the tattoo design, placement, and size have been discussed and agreed upon with the tattoo artist. I understand that changes made after the start of the tattooing process may incur additional charges and could impact the final outcome of the tattoo. I understand that tattoo aftercare is crucial for proper healing and that failure to follow the provided aftercare instructions may result in complications or undesirable healing outcomes. I accept full responsibility for the aftercare of my tattoo. I release the tattoo artists, and Mayhem Ink Tattoo Phuket, from any liability for any damages, losses, injuries, or claims arising from the tattooing process or the aftercare instructions, except in cases of negligence or willful misconduct. I acknowledge that the final appearance of the tattoo may vary depending on factors such as skin type, skin tone, and individual healing characteristics, I agree to allow Mayhem Ink Tattoo Phuket, to use photographs or images of my tattoo for promotional purposes, including but not limited to social media, website, or portfolio displays, while respecting my privacy and anonymity if requested.
